If you are a MOC-Floyd Valley or Unity Christian High School senior interested in pursuing a trade school, certificate program, or 2-year degree, please fill out this application for our $500 memorial gift by April 30th. Upon submission your responses will be sent to Tom's family, who will select one winner from each high school. You will be contacted via the email and/or phone number you provided.Â
